Survey as an E-Evaluation tool

How do I gather information in a short period of time? I used to ask this question a lot, and then I found the solution when my instructor mentioned the purpose of the survey.
I developed with the help of my friend Siwar Aridi this survey at the class of e-evaluation to obtain how teachers use technology and benefit from it in their classes.
